Use your Membership Number to save on registration for the Canadian Stroke Congress/Vascular 2013
This year’s Stroke Congress will be unique as it is part of Vascular 2013 – a one-time event that brings Canada’s four key medical meetings under one roof. Stroke Congress participants can register at a reduced member rate by using their membership number.
Registration will be divided into two categories: Members and Non-Members.
You are eligible for the reduced Member Registration Rate as you have attended one of the key stroke meetings listed below:
- Canadian Stroke Congress
- Ontario Stroke Collaborative
- Quebec Stroke Summit
If you are a part of one of these organizations, you will have received an email with your unique membership number. Please use this number when you register for Stroke Congress.

Delegate registration includes:
- Admission to the scientific programs, poster sessions and exhibition (Community Forum);
- Access to industry co-developed accredited symposia;
- Official Vascular 2013 delegate bag;
- Combined program-at-a-glance of all host organizations’ programs;
- Final Program – Electronic and/or Hard copy**;
- Vascular 2013 at your fingertips with the mobile app for iOS and Android smartphones and tablets
- Daily poster reception in the Community Forum (Thursday, October 17 to Saturday, October 19); and
- Buffet lunch and health breaks in the Community Forum from Thursday, October 17 to Saturday, October 19.
**You must indicate at time of registration if you wish to receive a hard copy of the Final Program.

Payment Policy
Registration fees must be paid in full by credit card (VISA, MasterCard or American Express) at time of registration.
Membership Number and Status
You will be required to input your membership number as part of the registration process. Membership status is verified and in the event your membership is not current, non-member registration rates will apply. The CSN does not refund the difference between member and non-member rates if you become a member following your registration for Vascular 2013. To locate your CSN membership number or verify your membership status, contact Membership Services.
Proof of Status
*Required for Full-time Students, Trainees and Residents.
Proof of status must be provided within 72 hours of submitting registration. Your registration is considered incomplete until your proof of status is received and verified.
Acceptable Proof of Status
• Active student card
• Recent transcript
• Letter from your supervisor or Dean

Registration Cancellations and Refunds
Until September 23, 2013
Requests for cancellations will be accepted in writing by the Secretariat until September 23 and will be subject to a $50 administration fee (plus applicable taxes).
After September 23, 2013
• Registration fees are non-refundable.
• Cancelled registrations are non-transferable.
• Cancelled activities are non-refundable.
• Guest cancellations are subject to a $25 administration fee (plus applicable taxes).
In the event of cancellation or non-attendance due to an emergency, submit your request in writing (along with supporting documentation) to the Secretariat no later than November 15, 2013. Requests received after this date will not be considered. Requests will be reviewed and evaluated on a case by case basis.
Delegate Substitutions
Until September 23, 2013 (without penalty)
Return to your original registration account by entering your reference number located at the top right corner of your confirmation e-mail.
After September 23, 2013 (with penalty)
Requests for delegate substitutions will be accepted in writing by the Secretariat and will be subject to a $50 administrative fee (plus applicable taxes).
Photo/Video Reproduction
The Vascular 2013 host organizations reserve the right to use any photo or video images recorded at Vascular 2013. By registering for Vascular 2013, you hereby acknowledge and agree that the Vascular 2013 host organizations may photograph and/or video you at this event, as well as use the photographs and/or videos in any publication or media for future educational and promotional activities/materials, without further notification or any compensation to you. For example, the selected images will assist in the promotion of future host organization events and may be used in professional displays, advertisements, printed publications, and/or on any of the host organization websites. You also acknowledge and agree to waive any right to inspect or approve any future educational and promotional activities/materials that may include photographs and/or videos of you.

All registration fees are subject to 5% GST, plus 9.975% QST for Québec residents.
Delegates who have attended one (or two) of the following conferences are eligible for member rates and will be sent an email containing a link and membership number. If you have not received this email, please contact Membership Services or call 613-562-5800 ext 8318.

Prior to registering, please take a moment to review the Preliminary Program for session details. As you register, you will be asked to indicate which sessions you intend on attending for space planning purposes.
